Meridian Regional Diagnostic Group LLC ("Meridian") is committed to protecting the privacy and security of protected health information ("PHI"). This Notice applies when Meridian acts as a covered healthcare provider. It does not replace a referring provider’s separate notice.
Your rights
Inspect and obtain copies
You may ask to inspect or obtain an electronic or paper copy of PHI maintained in a designated record set, subject to limited exceptions. We may charge a reasonable, cost-based fee when permitted by law.
Request correction
You may ask us to amend PHI you believe is incorrect or incomplete. We may deny the request in certain circumstances and will explain a denial in writing.
Request confidential communications
You may ask us to contact you in a particular way or at a particular location. We will accommodate reasonable requests as required by law.
Request restrictions
You may ask us to limit certain uses or disclosures. We generally are not required to agree, except when disclosure is to a health plan for payment or healthcare operations, the information concerns an item or service paid in full out of pocket, and other legal requirements are met.
Receive an accounting of disclosures
You may request a list of certain disclosures of your PHI made during the period permitted by law. The list does not include every disclosure, such as many disclosures for treatment, payment, or healthcare operations.
Obtain this Notice and choose a representative
You may obtain a paper copy of this Notice at any time. A legally authorized representative may exercise your rights when Meridian receives appropriate documentation of that authority.
File a complaint
You may complain to Meridian using the contact information below or to the U.S. Department of Health and Human Services Office for Civil Rights. Meridian will not retaliate against you for filing a complaint.
How we may use and disclose PHI
We may use or disclose PHI without written authorization when permitted by HIPAA, including:
- Treatment: to perform testing and communicate results or other information to healthcare professionals involved in your care;
- Payment: to bill, obtain payment, verify coverage, coordinate benefits, and manage claims;
- Healthcare operations: for quality assessment, compliance, accreditation, auditing, training, laboratory management, and other operations;
- Business associates: to vendors performing services involving PHI under contracts requiring appropriate protection;
- Public health and safety: for disease reporting, product recalls, adverse-event reporting, or preventing a serious threat as permitted by law;
- Legal and governmental purposes: for health oversight, workers’ compensation, law enforcement, judicial or administrative proceedings, coroners, medical examiners, organ donation, and other purposes permitted or required by law; and
- Research: when authorized or when an applicable legal waiver or other HIPAA permission applies.
Other uses or disclosures will be made only with your written authorization when required. You may revoke an authorization in writing, except to the extent we already relied on it.
Our responsibilities
- Maintain the privacy and security of PHI as required by applicable law;
- Use appropriate administrative, physical, and technical safeguards for electronic PHI;
- Notify affected individuals following a breach of unsecured PHI when required;
- Follow the privacy practices described in the Notice currently in effect; and
- Provide or make this Notice available as required by law.
Changes to this Notice
We may change this Notice and make the revised Notice effective for PHI we already maintain and information received in the future. The current version will be available on our website and upon request.
